These forms can be downloaded to your computer so you can type into them. Or, you can print them out and write on them.

Step 1 - Reading the Adam Walsh letter

  • Please start by reading the Adam Walsh letter which explains the need for fingerprinting and the procedure for having your fingerprints taken.

Step 2 - Application for Family Home Care

  • Next, fill out the Application for Family Home Care.

Step 3 - Criminal History Form

  • Fill out the form that fits your region
  • Snohomish County, it is region 3
  • King County is region 4
  • Pierce County is region 5
  • Thurston County is region 6
  • You will need to fill out a Criminal History Form for each person in your household that is 16 years old and older.

Step 4 - Region 5 Address History form

  • You will need to fill out one for each member of your household that is 16 years old and older
  • This form works for all of the regions.
